What is Geriatric Care Management?
Geriatric Care Management focuses on caring for our elderly patients’ health and wellness. This includes physical, mental, and social needs. We partner with our older patients to proactively prevent diseases and disabilities, improve their quality of life, and help maintain their independence. Common medical conditions we treat or help manage include heart disease, arthritis, high blood pressure, memory related issues like dementia, diabetes, incontinence, hearing or vision loss, and much more.
