What is Chronic Care Management?
Chronic Care Management is a service provided to our senior patients with Medicare coverage. The Centers for Medicare & Medicaid Services define Chronic Care Management patient eligibility as “Patients with multiple (two or more) chronic conditions expected to last at least 12 months or until the death of the patient, and that place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline.”* Some examples of chronic conditions may include, but are not limited to, arthritis, atrial fibrillation (a-fib), cardiovascular disease, chronic obstructive pulmonary disease (COPD), or diabetes.
At Pak Medical Group, we work closely with our elderly patients to provide expert care and continue to improve their quality of life. Our Chronic Care Management includes meeting with you to discuss our care plan and goals, providing 24/7 access to our physicians or a member of our clinical team for urgent needs, preventative care services, treatment of existing conditions and a comprehensive treatment plan, medication management, and the coordination and management of care between our team and other health care providers or facilities.
